Catholic Health Services offers Financial Assistance to those patients and their families who qualify based upon total household annual income and family size according to the most recent Federal Poverty Guidelines (FPG).

For example, income below 200% of the FPG would be a family of four with a household income of $51,500 or less would be eligible for free medical care, rent/tuition.

Our Mission

In accordance with Catholic Health Services mission to provide health care and services to those in need, to minimize human suffering, to assist people to wholeness and to nurture an awareness of their relationship with God, it is the policy of Catholic Health Services to offer financial assistance for medical bills to our patients and their families based on the ability to pay. We will make every effort to accommodate financial assistance requests, however, we are limited by our financial resources.

Financial Assistance Eligibility

We are proud to offer a financial assistance program, designed to work directly with patients/residents to find payment solutions when necessary.

Based on the Federal Poverty Guidelines, financial assistance discounts are determined by a sliding scale of total household income. See-Attached-Federal-Poverty-Income-Guideline-Sliding-Scale-Assistance-Policy.

      Simple Application Process

The individual facilities which comprise Catholic Health Services and their financial counselors and medical assistance representatives can assist you in identifying assistance programs which may aid patients and their families with payment of their healthcare charges, rent, and tuition charges.

Eligibility for financial assistance for medical bills is determined by the patient’s/resident’s or his/her guarantor’s ability to pay after all available resources have been utilized and all available assistance programs have been accessed. The program covers payment for medically necessary care, rents, and tuitions, but does not cover routine co-pays and deductibles for patients/residents having medical coverage unless a hardship/need can be documented. Submission of all income and expenses, including medical expenses, plus assets and liabilities of the patient and or guarantor is required when requesting financial assistance.

Guidelines

Financial assistance for medical bills programs are available for the medically indigent, uninsured, underinsured, or financially needy individuals or families of U.S. citizens and legal U.S. residents.

Emergency Medical Care Policy

Facilities do not have dedicated emergency departments, nor do they have specialized capabilities that would make it appropriate to accept transfers of individuals who need stabilizing treatment for an emergency medical condition, as such we do not have a Provider list. Facilities follow written emergency medical care policies that address how to appraise emergencies, provide initial treatment and refer or transfer an individual to another facility when appropriate. Facilities medical care policy also prohibits facilities from engaging in actions that discourage individuals from seeking emergency medical care, such as demanding that patients pay before receiving initial treatment of emergency medical care. Copies of emergency medical care policies, which include First Responder, Code Blue, Cardio-Pulmonary Arrest/Resuscitation can be found at www.catholichealthservices.org.

Billing and Collection Policy

The organization has a separate billing and collection policy. The hospital does not engage in extraordinary collection actions against an individual to obtain payment before the hospital facility has made reasonable effort to determine whether the individual is eligible for assistance for the care provided under the Financial Assistance Policy. Medicaid Beneficiary

  • An approved Florida Medicaid beneficiary who receives services in our facilities which are not covered by this program automatically qualifies for financial hardship/charity assistance of non-covered healthcare balances.

Financial Assistance – Examples

  • Income below 200% FPG – A family of four with a household income of $51,500 or less is eligible for free medical care, rent/tuition.
  • Income above 200% but below 400% – Under Catholic Health Services’ Financial Assistance Policy, medical bills, rents and tuitions and fees are capped at 20% of total household income; therefore, a family of four, making $103,000/year, with a $100,000 bill, should expect to be personally responsible for no more than $25,200 annually.
  • In no event will a patient be charged more than the amount generally billed in accordance with Section 501r(5) which, based on the look-back method, is our net revenue realized as a percent of gross charges (59%) for all payors.
  • Other than the hospital staff, contracted Physiatry physicians provide medically necessary care and are covered by the hospital facility’s Financial Assistance Policy.

Application Process

The application process is easy. Simply print the Verification Letter and complete the Application for Financial Assistance form in English or Spanish and fax, mail or deliver to your facility’s Business Office along with all supporting documentation requested. A financial counselor will contact you once your information has been received. (Please note that proof of income or other financial support is required to qualify. The provision of inaccurate information or the failure to provide requested supporting documentation will serve to disqualify the applicant from receiving financial assistance.)

Catholic Health Services contributed almost $12 million in community benefit in the fiscal year to the communities it serves in Broward and Miami-Dade counties in south Florida. In reporting its community benefit, Catholic Health Services follow the guidelines set by the Catholic Health Association.

Making a difference in South Florida is a primary part of our mission. As the largest post-acute health care system in the southeast United States, Catholic Health Services has provided over $10 million in community benefit services each year. To those who may have difficulty in accessing health care services, we have many outreach programs. To those who may not be able to afford the cost of healthcare, we assist and provide resources or financial assistance. To those who may not be knowledgeable about their own health issues, we provide education. To those who need a little extra help in managing their daily lives, we provide a helping hand. To those who are suffering the loss of a loved one or preparing for a loss, we provide compassion and spiritual support. To those elderly living with low income, we provide safe and dignified housing. Through our various service lines, both in health care and in social services, we strive to make all transitions of care as smooth as possible for patients and their families, recognizing that many of them are very vulnerable and often overwhelmed by existing systems.

Our programs, many of which have been nationally recognized, ranging from health education and free screenings to clinical and ancillary medical services training to doctoral level internships in our rehabilitation hospitals. Community partnerships vary from affiliation with local universities, colleges and high schools to advocacy groups for awareness, campaigns, screenings, workshops and support groups.

Archbishop Thomas G. Wenski, born in West Palm Beach on October 18, 1950, grew up in Lake Worth, Florida where he attended Catholic school at his home parish, Sacred Heart. He studied at St. John Vianney Minor Seminary in Miami and St. Vincent de Paul Major Seminary in Boynton Beach and was ordained a priest of the Archdiocese of Miami on May 15, 1976. He has a Bachelor of Arts degree in Philosophy (1972) and a Master of Divinity (1975) from the Boynton Beach Seminary and a Master of Arts in Sociology from Fordham University in New York.

He served three years as associate pastor of Corpus Christi Church, a mainly Hispanic parish in Miami, before being assigned, in 1979, to the newly established Haitian Apostolate of the Archdiocese. He was the associate director and then director of the Pierre Toussaint Haitian Catholic Center in Miami’s Little Haiti section while serving concurrently as pastor of three Haitian mission parishes in the archdiocese: Notre-Dame d’Haiti in Miami, Divine Mercy in Fort Lauderdale, and St. Joseph in Pompano Beach. Throughout the early 1980s, he also conducted a circuit-riding ministry to Haitian Catholic communities as far south as Homestead, as far north as Fort Pierce, and as far west as Immokalee.

In January 1996, then Father Wenski was appointed the Archdiocese Director of Catholic Charities, one of the largest Catholic social service agencies in the United States. In this capacity, he helped forge a collaborative relationship with Caritas Cuba, the social service arm of the Catholic Church in Cuba. Since early 1996 he has traveled to Cuba on many occasions on behalf of the Church.

Appointed Auxiliary Bishop of Miami on June 24, 1997, he was ordained to the episcopacy on September 3, 1997. Pope John Paul II appointed Bishop Wenski as coadjutor bishop of the Diocese of Orlando on July 1, 2003, and he assumed the role of the fourth bishop of Orlando on November 13, 2004. On April 20, 2010, Pope Benedict XVI appointed him the fourth Archbishop of Miami and Metropolitan of the Province of Miami (which includes the seven dioceses of the State of Florida). He was installed as Archbishop of Miami on June 1, 2010.

Ralph Lawson Head Shot 2010Ralph Lawson is the Executive Vice President and Chief Financial Officer of Baptist Health South Florida, Inc. Baptist Health is the largest, not-for-profit healthcare organization in South Florida, comprised of seven hospitals and more than 30 outpatient facilities spanning three counties. With more than 16,000 employees and 2,200 physicians, Baptist Health is considered one of the nation’s top employers, according to well-known workplace surveys such as Fortune magazine’s 100 Best Companies to Work For. In addition, Baptist Health hospitals were recognized for high quality and patient safety with numerous national and regional awards from U.S. News & World Report.

Prior to joining Baptist Health, Mr. Lawson spent 17 years at Deloitte, the last eight years of which were as a partner responsible for the Florida healthcare practice.

Mr. Lawson earned a Bachelor of Science Degree from the University of Colorado and a Master’s Degree in Business Administration from The Wharton School of Finance, University of Pennsylvania.

Mr. Lawson also serves as Chairman of the Board of Catholic Health Services, which employs over 2,500 people. Catholic Health Services is the largest post-acute continuum of care provider in the southeast United States, caring for over 6,500 people of all faiths on a daily basis, through eight different service lines in healthcare and social services.

Ralph Lawson is a past National Chair of the Healthcare Financial Management Association (HFMA). HFMA has over 40,000 members and 68 Chapters throughout the United States. HFMA is one of the most recognized sources for unbiased information as to how to reform and improve the United States healthcare system.

Mr. Lawson is the National Secretary/Treasurer of the Board of Directors of the Leukemia & Lymphoma Society (LLS).

Ralph Lawson is a well known U.S. healthcare executive. He is a frequent speaker at industry and civic meetings throughout the country and has been featured on the cover of Modern Healthcare magazine.

Ralph is married to Barbara Lopez Lawson and resides in Parkland, Florida. They have five children and 4 grandchildren.

Pastoral and spiritual care services are integral to the healthcare and services of the Catholic Health Services organization. Pastoral and spiritual care offers healing, sustaining, guiding, reconciling and counseling services. Pastoral care ministry is provided to patients/residents, their families/community, and staff. Pastoral Care is provided to patients, their families, and staff; giving spiritual support with respect for each person’s faith, tradition, and spiritual perspective and functioning in an interdisciplinary manner with other healthcare colleagues.

Because the human person is by nature spiritual, pastoral care seeks to honor and facilitate the search for meaning and integrity in the experiences of life including sickness, death, and bereavement, by offering presence, compassion, listening, support and education.

Dr. Daniel C. Carney, DO

Dr. Carney received his undergraduate degree (BS) from the University of Miami in 1977. His path to Rehabilitation Medicine was, however, a bit unconventional. A former sportscaster and talk show host in Miami and in Atlanta for CNN, Dr. Carney realized the beginning of his lifelong passion by attending Nova Southeastern University College of Osteopathic Medicine. Graduating in 1995, he completed his internship at Palmetto General Hospital and then moved to Boston, Massachusetts for his residency in Physical Medicine and Rehabilitation at Boston University Medical Center. Upon completion in 1999, he joined the staff of Boston University Medical School as Assistant Professor of Rehabilitation Medicine. He is Board certified in Physical Medicine and Rehabilitation and attained his Subspecialty in Spinal Cord Injury from the American Board of Physical Medicine and Rehabilitation in 2002. Dr. Carney is also Board Certified by the American Osteopathic College of Physical Medicine and Rehabilitation. He is licensed to practice medicine in Florida and Massachusetts. Dr. Carney is a native of South Florida, and it was only natural that he and his family return to the Sunshine State in 2003. He has served as Medical Director of St. Anthony’s Rehabilitation Hospital since that time. His clinical interests include spinal cord injury, stroke, sports medicine, and musculoskeletal disorders. Dr. Carney is married with 4 children, and enjoys golf, fishing, and antique “muscle” cars.

Dr. Manish M. Patel, M.D., F.A.A.P. M&R
Dr. Patel joined the Catholic Health Services Medical Group after having his own private practice and private group practice for many years.  His practice consisted of an inpatient rehabilitation unit, several skilled nursing facilities and an outpatient office practice specializing in Pain Management, Electromyography, Acupuncture and Independent Medical Examination. Graduating with honors from Municipal Medical College in 1989, Dr. Patel stood at third rank in Gujarat University in India in the final M.B, B.S. examination.  He was also listed among five candidate s for “Best Outgoing Students for Year 1991” in Smt. N.H.L. Municipal Medical School, India.  Dr. Patel is a member of the American Academy of Physical Medicine and Rehabilitation and the American Academy of Pain Management.  He has also received certifications in Educational Commission for Foreign Medical Graduates (ECFMG).   Dr. Patel was also the Medical Director for an Inpatient Rehabilitation Unit at Memorial Regional Hospital in Hollywood, Florida before going into his own private practice.  Prior to that, he was the Chief Resident at Kingsbrook Jewish Medical Center in Brooklyn, NY. Dr. Patel is licensed to practice medicine in the State of Florida.  Married for many years, Dr. Patel has two children.  The family enjoys spending time together and traveling.  When Dr. Patel is not with his family, he enjoys a great game of tennis.

Dr. Lina Hurtado, M.D.
Dr. Hurtado joined the Catholic Health Services Medical Group July 2015.  She started her career at Central Military Hospital in Bogota, Colombia as an intern.  Dr. Hurtado then received her medical degree from Nueva Granada Military University, Bogota, Colombia in 2001.She has also received her Educational Commission for Foreign Medical Graduates in 2007 in Philadelphia, Pennsylvania in 2007.  Dr. Hurtado is experienced in providing quality health care services to patients from a wide variety of socio-economic backgrounds.  She is bi-lingual and she has proven her ability to clinically diagnose and treat ill patients. Her post graduate training was held at Jackson Memorial Hospital, Miami, Florida where she held internships in the Kidney and Pancreas Transplant Department and in the General Surgery Department.  She completed her residency in June 2015 at Jackson Memorial Hospital/University of Miami, Physical Medicine and Rehabilitation Department. Dr. Hurtado has also received her certification for Advanced and Basic Life Support in April 2013.  Dr. Hurtado is married and enjoys traveling and exercise.

Dr. Miriam Feliz, M.D.
Born in Santo Domingo, Dominican Republic, Dr. Feliz received her undergraduate and medical school education at the Autonomous University of Santo Domingo, graduating in 1975. She undertook internship training in internal medicine at the Moscoso Puello General Hospital in Santo Domingo (1973-1975). She completed her residency training in Physical Medicine and Rehabilitation at Boston University Medical Center (1977-1980). She also completed a fellowship in industrial rehabilitation and spinal cord injury at Boston University Medical Center (1980-1981). She became Board certified in Physical Medicine and Rehabilitation in 1989. She is licensed to practice medicine in Florida, Massachusetts, New Jersey, and Washington, DC. Dr. Feliz established her medical practice in 1982 in the specialty areas of Physical Medicine and Rehabilitation, Geriatric Rehabilitation, Industrial Medicine, and Spinal Cord Injury. In 1988, she began a now long-standing commitment to St. Catherine’s Rehabilitation Hospital. Presently, she is a Medical Director at St. Catherine’s. Her current interests include cardiac rehabilitation, brain injury, and osteoporosis. Her favorite hobby is boating and spending time with her family. Dr. Feliz “retired” effective August 2015 and decided to come back to work as part of  the Catholic Health Services Medical Group in December of 2015 as one of  the medical directors for St. Catherine’s West Rehabilitation Hospital.

SUMMARY OF THE FLORIDA PATIENT’S BILL OF RIGHTS AND RESPONSIBILITIES

Florida law requires that your health care provider or health care facility recognize your rights while you are receiving medical care and that you respect the health care provider’s or health care facility’s right to expect certain behavior on the part of patients. You may request a copy of the full text of this law from your health care provider or health care facility. A summary of your rights and responsibilities follows:

  • A patient has the right to be treated with courtesy and respect, with appreciation of his or her individual dignity, and with protection of his or her need for privacy.
  • A patient has the right to a prompt and reasonable response to questions and requests.
  • A patient has the right to know who is providing medical services and who is responsible for his or her care.
  • A patient has the right to know what patient support services are available, including whether an interpreter is available if he or she does not speak English.
  • A patient has the right to know what rules and regulations apply to his or her conduct.
  • A patient has the right to be given by the health care provider information concerning diagnosis, planned course of treatment, alternatives, risks, and prognosis.
  • A patient has the right to refuse any treatment, except as otherwise provided by law.
  • A patient has the right to be given, upon request, full information and necessary counseling on the availability of known financial resources for his or her care.
  • A patient who is eligible for Medicare has the right to know, upon request and in advance of treatment, whether the health care provider or health care facility accepts the Medicare assignment rate.
  • A patient has the right to receive, upon request, prior to treatment, a reasonable estimate of charges for medical care.
  • A patient has the right to receive a copy of a reasonably clear and understandable, itemized bill and, upon request, to have the charges explained.
  • A patient has the right to impartial access to medical treatment or accommodations, regardless of race, national origin, religion, handicap, or source of payment.
  • A patient has the right to treatment for any emergency medical condition that will deteriorate from failure to provide treatment.
  • A patient has the right to know if medical treatment is for purposes of experimental research and to give his or her consent or refusal to participate in such experimental research.
  • A patient has the right to express grievances regarding any violation of his or her rights, as stated in Florida law, through the grievance procedure of the health care provider or health care facility which served him or her and to the appropriate state licensing agency.
  • A patient is responsible for providing to the health care provider, to the best of his or her knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to his or her health.
  • A patient is responsible for reporting unexpected changes in his or her condition to the health care provider.
  • A patient is responsible for reporting to the health care provider whether he or she comprehends a contemplated course of action and what is expected of him or her.
  • A patient is responsible for following the treatment plan recommended by the health care provider.
  • A patient is responsible for keeping appointments and, when he or she is unable to do so for any reason, for notifying the health care provider or health care facility.
  • A patient is responsible for his or her actions if he or she refuses treatment or does not follow the health care provider’s instructions.
  • A patient is responsible for assuring that the financial obligations of his or her health care are fulfilled as promptly as possible.
  • A patient is responsible for following health care facility rules and regulations affecting patient care and conduct.

Catholic Health Services Privacy Policy

Catholic Health Services (“CHS”, “we”, or “us”) operates the web site located at https://www.catholichealthservices.org (the “Site”). This serves as the Privacy Policy for the Site and CHS events.

CHS makes the Site and various offers related to CHS and/or the Site available to those individuals who register directly with the Site or at CHS events (“Registrants”), subject to this Privacy Policy (the “Policy”). This Policy explains what information CHS collects about its Registrants, how CHS uses and/or shares this information, and how such information is maintained. By visiting the Site, or by registering on the Site, you signify your acceptance of this Policy.

Note that this Policy applies only with respect to the information collected by CHS and not any information collected or obtained through other methods or sources.

Collection of Personally Identifiable Information

CHS collects Registrants’ personally identifiable information (“PII”) that is volunteered by Registrants during the registration process or in response to specific information requests explicitly presented to Registrants. Examples of PII that may be requested and/or collected include: first and last name, title, address, email address, telephone number, gender, age, education, and household income. From time to time CHS may also present opportunities for Registrants to voluntarily provide additional information about themselves. CHS video records and photographs its events for posting on the Site and other sites. Registrants at events are recorded in those videos and photographs.

Use and Sharing of PII

CHS reserves the right to use Registrant PII: (1) for the specific purpose for which such information was provided, for example on the Site, in order to be a subscriber to the CHS email newsletter; (2) as disclosed at the time such information is provided; and/or (3) as disclosed in this Policy. However, please note that CHS may change this Policy at any time (See “Changes to this Policy,” below), and that therefore CHS may in the future use Registrant PII for additional purposes not currently included in this Policy.

Registrant PII collected by us may be added to our databases and used for future marketing purposes. By using the Site, you agree that you may be contacted in any manner contemplated in this section even if your number is found on a do not call registry, in-house list or similar registry.

From time to time CHS contracts with third parties to conduct marketing promotions, sweepstakes and other promotions. The PII collected from such promotions may be used for any purpose disclosed in this policy or in the rules to the specific promotion.

CHS may share Registrant PII in aggregate, non-personal form, with select companies that wish to reach a particular Registrant target audience.

CHS may disclose Registrant PII in order to: (1) comply with applicable laws (including, without limitation, the CAN-SPAM Act); (2) respond to governmental inquiries; (3) comply with valid legal process; and (4) protect the rights or property of CHS, including without limitation, filing copyright applications with the Library of Congress, Copyright Office.

CALIFORNIA RESIDENTS: YOUR PRIVACY RIGHTS

California Civil Code Section 1798.83, requires companies that collect PII from California residents and disclose such information to third parties (including affiliated entities) for marketing purposes to allow California residents to provide a mechanism by which a consumer may opt-out of having his/her information shared with third parties. Non-Personally Identifiable Information

CHS also collects certain non-personally identifiable information, including the non-personally identifiable information more fully described below.

Cookies

CHS may use “cookies” on the Site. Cookies are small pieces of information that CHS’s server places on an individual’s computer hard drive to enable the individual to more easily communicate and interact with the Site. For example, CHS may place a cookie on a Registrant’s computer when he/she initially clicks on a link in an email from CHS. The next time that Registrant opens an email from CHS and clicks on the link, CHS’s server will recognize the cookie (and the Registrant) and allow the Registrant to perform certain actions immediately without signing in.

While some Web browsers allow a Registrant to automatically reject all cookies or be notified when a cookie is about to be placed on the Registrant’s computer, disabling this feature may compromise the efficiency and added services that cookies provide.

IP Address

CHS collects your IP (Internet Protocol) address to help diagnose problems with its server, and to administer the Site and CHS email offers. An IP address is a number that is assigned to your computer when you use the Internet. Your IP address is also used to help identify you during a particular session and to gather broad demographic data.

Although the information collected through the foregoing methods does not itself contain any PII, CHS may analyze and match such information with other information that you provide (including PII) as well as information that CHS may obtain elsewhere, and CHS may share all or some of such information with CHS may also disclose non-personally identifiable information in order to (1) comply with applicable laws, (2) respond to governmental inquiries, (3) comply with valid legal process, or (4) protect the rights or property of CHS or its Registrants and users of the Site.

Opt-Out of CHS Media Promotions

During registration, Registrants will have the option to not receive promotional offers and updates, via emails, from CHS and our partners. In order to not receive such promotional offers and updates, Registrants must elect that option, where indicated.

If a Registrant chooses to receive promotional offers and updates, Registrant may, at any time thereafter, choose to discontinue receiving promotional emails from CHS or the Site by following the “unsubscribe” instructions in the applicable email. Notwithstanding the foregoing, CHS may continue to contact Registrants for the purpose of communicating information about their registration with CHS or similar information relating to CHS, the Site or their registration, as well as to facilitate, complete or confirm any transaction that Registrants enter into or in response to or fulfillment of Registrants’ inquiries or requests.

CHS provides links to many of CHS’s advertiser and partner websites and other websites. If you link to any of these third party websites, this Policy will not apply to your use of, and activities on, those websites. CHS shall have no responsibility or liability for your visitation to, and the data collection and use practices of, these websites.

Note Concerning Individuals 13 Years of Age And Younger

The Site, CHS emails and the content available in connection therewith, are neither intended for, nor directed to, children under the age of 13. You must be at least 13 years old to have our permission to use our websites and to provide your PII to CHS. If CHS learns that a person who is a Registrant is under the age of 13, CHS will promptly terminate that individual’s registration and delete the individual’s PII from CHS’s records. Security

While CHS takes reasonable precautions to safeguard information transmitted between CHS and Registrants and/or users of the Site, CHS may be unable to prevent unauthorized access to such information by third parties or inadvertent disclosure of such information. Registrants and users acknowledge this risk when communicating with CHS.

Consent to Processing

By providing PII and other information to CHS, Registrants of the Site fully understand and unambiguously consent to the collection and processing of such information in, and the transfer of such information to, the United States and other countries or territories, in accordance with the terms of this Policy.

Transfer in Certain Circumstances

In its sole discretion, CHS may transfer, sell or assign information collected on or about Registrants of the Site, including without limitation, PII and other Registrant provided information, to one or more third parties, as a result of the sale, merger, consolidation, change in control, transfer of substantial assets, reorganization or liquidation of CHS.

If any provision of this agreement is found by a court of competent jurisdiction to be invalid, the parties nevertheless agree that the court should endeavor to give effect to the parties intentions as reflected in the provision, and that the other provisions of this agreement remain in full force and effect.

The Terms and Conditions and the relationship between You and Catholic Health Services and the Site shall be governed by the laws of the State of Florida without regard to its conflicts of law provisions. You and the Site agree to submit to the personal and exclusive jurisdiction of the Courts of the Fifteenth Judicial Circuit of Florida in and for Broward County or the U.S. District Court for the Southern District of Florida.  You agree that regardless of any statute or law to the contrary, any claim or cause of action arising out of related to the use of the Site or Terms and Conditions must be filed within one (1) year after such or cause of action arose or be forever barred. These Terms and Conditions as well as the Privacy Policy, which is incorporated by reference, constitute the entire agreement between you and Catholic Health Services for the use of its Site.
